We employ expert clinicians who have training and experience to diagnose your musculoskeletal issue, identify the most beneficial treatment, and guide you to recovery in the fewest visits possible. Most PT clinics want you to come 2-3 days a week for 8-12 weeks and you will work with technicians and PT assistants. Here, at The Perfect Squat, you will work 1-on-1 with a board-certified orthopedic specialist and doctor of physical therapy for your full session, every session. We use evidence-based treatments and equip you with knowledge and techniques, which leads to less frequent and often less visits.

No, the state of Indiana Practice Act for Physical Therapists allows 42 days of open access. If your treatment will require more time or if your therapist deems it necessary, then she may ask you to obtain a physician referral.

Your physical therapist will give you a recommendation based on evidence and experience; however, it may be longer or shorter than initially anticipated.

The Perfect Squat works to provide expert care tailored to your needs. Our therapists work together with you to create a plan that optimizes recovery and function for you, the client. Because of this we are not your standard physical therapy clinic and we are considered “Out of Network”; we are not connected to insurance companies. We do accept payment via HSA or FSA accounts. Receipts are provided to all those who request them at time of service. Detailed receipts include all necessary billing codes to allow you to send your claim for reimbursement to your insurance company.

Group coaching is a small group class where 2-6 people come during dedicated hours to do their workout for the day. The coach is available to assist with form, offer suggestions, and answer any questions. Exercising in a group provides accountability and motivation to help you achieve your health and wellness goals!

Insurance companies dictate patient care based on algorithms, rates of reimbursement, pre-authorization and other, non-individualized means. At The Perfect Squat we strive to provide you the best plan for you; therefore, we do not contract with insurance companies and we are considered an “out of network” provider. We do encourage you to consider the costs and benefits of working with The Perfect Squat versus a therapy clinic that takes your insurance. Check with your insurance plan, most do cover physical therapy, but often you must meet your deductible first and you often have a copay. Traditional therapy clinics will take your co-pay up front and then bill insurance for the remainder which ranges anywhere from $300-$700. If you have yet to meet your deductible, then you will receive a bill 6-8 weeks after you started therapy and accrued 12-24 visits costing $3,600-$7,200. It adds up quickly! Often clients pay less with “out-of-network” rates than one would when using insurance due to high deductibles and copays.
We want to work together with you, the client, to provide our best recommendations for an effective, efficient, and functional recovery and that is why we chose to go out-of-network.

The Perfect Squat is not a participating Medicare provider. Due to US laws, we can only accept Medicare beneficiaries who DO NOT want Medicare billed for physical therapy services. If you are a Medicare Beneficiary, you must request up front and of your own free will that you do not want Medicare billed for physical therapy. If you are determined to see The Perfect Squat for physical therapy, knowing we are not enrolled as a provider with Medicare and that you do not want Medicare billed for services, then we can see you. If you choose to use your Medicare benefits, then we can suggest Medicare enrolled physical therapy providers near you.
